Both him and Tony had been in denial up to this point, and that’s why it took that doctor talking to Tony just before that scene for him to accept it. They both know that they love each other, but in that moment they are finally accepting the reality of what’s going on with Junior.

Jun literally put a hit on Tony and he forgave him and even came to him for advice for many things after that. They have a real close relationship ever since they were kids. They are trying to limit his liquid intake because of his bed wetting. Junior has never been the boss. He's a figurehead. He was positioned to be the focus for the Feds, but Tony has always been the Boss.
