Death Bell
Death Bells are thought to be the incarnation of negative emotions that come from the loss of many around them, coalescing in the herald of that death, the church belfry. While few have experienced such loss necessary to summon a being like this, during times of particularly deadly battles, they have been known to spawn.

This just showed up at literally the perfect time. A couple months ago I was running a campaign which ended with a church cult murdering one of the player characters and casting his corpse in bronze and grafting it to the church bell. The rest of the party is now on an adventure to discover who killed their comrade. This will make the perfect boss fight. I will probably need to beef it up a bit only because I want to fight to drag out. (Likely just adding a separate phase where it hangs from the rafters and summons its victims until they figure out to knock it down.) But otherwise this is perfect.
