In the world of TLOU, it's important that the characters and the audience believe that the cure was outcome of Ellie's surgery. Of course it's not realistic, so you have to suspend disbelief, but if you can believe in a world with mushroom zombies, then believing in a viable cure for the zombies isn't that much to ask. It was the creators' intent to make Joel's choice to choose Ellie over the cure more impactful. Without that conceit, his choice becomes much less meaningful. Arguing about the viability of the cure in TLOU is like arguing about public transportation safety measures in The Trolley Problem. You're missing the point. Joel doesn't save Ellie because he doubts the efficacy of the cure. He believes in it, but he saves her because he loves her more than he loves the rest of humanity. And Ellie doesn't open the door to forgiving Joel because she doubts the cure. She does it because she loves him despite the terrible sacrifice he made. It's beautiful in a way you just can't appreciate if you don't give the Fireflies the benefit of the doubt. If you want to critique the show for failing to make the audience believe in the cure, that's fair. But in terms of character motivations, belief in the cure is a must.
