Google Form for request stream ideas!

Follow the link for the form: and please let me know if anything needs clearing up or additional information. I may have forgotten something, but I explain on the form how it should work. When it comes time to pick a name/idea, I'll roll a number between 1 and whatever the maximum "accepted" idea number is, and pick that. Accepted meaning, if there are ideas and suggestions I won't do. I'll let people know if they make one, and delete it so they can submit again. (i think it should work)
But to prevent people submitting multiple ideas under different names, I've enabled the Google account sign-in "Limit 1 response" feature. I won't see your Google account name, just what you enter in the fields.
