The gunshot at the start is the end of the "Confessions" interlude, which I love. This performance was controversial among Christians. People accused her of impersonating Jesus, instead of focusing on the message that lines up with what Christianity preaches. This is one of those performances where Madonna made changes to the song from the studio version, the main one being the addition of the church organ, which made this be the best version of Live to Tell.

Two decades after the aids epidemic in North America but she is highlighting that it is still a problem especially in Africa because young people, and the western world, have forgotten about the aids epidemic and how serious it was at least to some degree. But no definitely not 80s or 90s lol
