Side quest where Gwen has to get the milk from the grocery store but the milk is guarded by Minamos, The Yggdrasil Cow, who has three heal bars and a massive AOE that takes out all your health.

But as soon as you mentioned Runes my mind immediately went to: Marriage and divorce in Elden Ring, high chance to lose ALL Runes π (As if FromSoftware games weren't depressing enough π€£)
