Mistranslation. And no, the mindset was entirely different. Blame yourself for not having a shred of media literacy tbh. Yuji has been on the verge of a breakdown since he switched back with sukuna, then nanami dies infront of him, next nobara dies and he's completely and utterly broken, lost the will to live and fight back entirely. Todo had to come in and literally rebuild yujis mental state from the ground up. So they're completely different mindsets, one was the mindset of yuji on a downward spiral as he loses control of everything and everyone he cares about is dying in front of him. The other is post mental break, entirely renewed resolve and he has todo there with him. Todo being there makes all the difference when, like we've seen before, todos encouragement has a profound effect on yuji, demonstrated by last time they fought together yuji tied the consecutive black flash record.

Todo can only switch things that have curse energy, this why you see them imbuing rocks with curse energy to teleport to them. The theory is that Todo can only switch things he can see. This is a bit iffy since he sometimes teleports things that are behind him so it may be the things that are within in a certain distance/or recently have been seen. This can explain why he doesn’t just teleport Mahito away.

It's basically a type of Damage Stasis ability, so while she has no pulse & isn't breathing it's not been that long of a time where she'd become braindead even w/ resuscitation attempts, so she's basially in stasis between a few seconds of blood loss to brain & the amount of seconds you need to have blood loss to the brain for you to have either heavy brain damage or death due to too long of a time between loss of blood & EMT. Landing a black flash can't be done 100% intentionally, it's more like it happens when you're reaaalllyy in the zone and everything lines up just right for you. It's not quite random, it's a result of skill and concentration (and a little bit of luck). After landing a black flash you acquire a heightened sense for and understanding of cursed energy. You also ride a high from it described as "a feeling of omnipotence" as if everything revolves around you, bringing out more of your potential.
