If you're asking why Ben didn't gain muscle mass, it's because in comparison to Ben 10K, he didn't completely stop being lazy (specifically for dating with Rook) and like Ben UA, Ben can have the powers/strength of his aliens too (can turn into aliens but can't fuse aliens).
Unlike Ben 10K, Ben stopped being a hero for a while and Gwen replaced him. Rook is thinner and beardless compared to Rook 10K due to dating with Ben, he takes more care of his appearance and works more as a Plumber and Magistratus for his family.
They won't look so old because BE GAY IS BE IMMORTAL 🤣kidding~ just, it's alternative future.
