I'm not wearing a bra under the nurse outfit, but instead, I've made it so that when my chest moves, my nipples are visible through the fabric. For the underwear, the bra shows through, allowing my nipples to be faintly visible. Which did you prefer? The nurse outfit where nipples peek through, or the underwear with minimal fabric where nipples are faintly visible?
