The liquid that alphas are producing needs to be injected, and Kacchan just got it on the face, so he’s still unclaimed. Kacchan’s parents bribed the doctor because they knew that if Kacchan won’t be able to achieve his ultimate goal (going to U.A. Which is impossible if you’re an omega) - only god knows what Kacchan would do to himself and/or others

Although Izuku was the first one to catch Kacchan’s scent in the class and almost told everyone about it without knowing who’s the owner of that scent. Kacchan is both angry and scared that someone might find out about his little secret. I hope it’s helpful ^^.
