Remember in 4x02, Kate is looking for it to learn control. It was her reason for bringing Derek back to his teen years. And we know it wasn’t in their family for hundreds of years because Peter says there’s a part scratched off in the back where “Made in China” used to be. So it’s nothing special, but yes it is used as a placebo. Also, remember when Peter says “Desert Wolf,” malia says it means coyote. So the desert wolf isn’t a wolf at all. (Hence malia being a werecoyote). That’s all I can say without giving anything that hasn’t been revealed already. Derek evolved from the normal werewolf transformation (human with wolf characteristics, like Scott, Liam, etc.) to being able to turn into an actual wolf which is to be a special ability revealed in 3x08. You can see that he had gotten stronger just from him singlehandedly taking down a berserker.
