I'm not sure if anyone mentioned this, but Unlimited Blade Works has only a part of the full story. The fate series has different routes, and you will also have to watch the three part Heaven's Feel movies to get the full picture.

Remember in the first scene of Zero she is shown to be born 8 years prior to the 4th war thus was 8 during the main bulk of fate zero and 18 here. She ages differently because she's half Homunculus. Likewise despite their looks the maids are less than a year old. Somethings not mentioned in the anime is Berserker was the first servant summoned for this war, having been summoned 2 months before. The Grail normally helps with the summoning but because they did so early it wasn't able to help in this instance. This made Illya and Berserker's contract extremely painful for Illya at first, with her feeling pain at Berserker's slightest movement. Likewise prior to the summoning she had command seals surgically implanted across her entire body which is why she has bandages on in the scene where she speaks to the other Homunculi and why her command seals show on her face, in the original visual novel you see them glowing through her clothes. The evil Iris manipulating Illya is anime original and whilst its not explained what exactly it is, it has the same appearance as what came out of the Grail at the end of fate zero. So it is likely some manifestation of the Grail. It can't be a hallucination because it tells her things Illya wouldn't be able to know like Kiritsugu adopting Shirou.
