I had answers about the Ghouls in this episode : they are people who got irradiated, and we don't care when, it's just a question of environment, too much exposition. Our cowboy managed to survive with these vitals all that time, to stay "alive", because without it, he can transform and just become that ki**er creature. But I would have loved for him to respond to Lucy's question about the "why", why would he want to stay alive, like that ? His wife and kid died a long time ago so… does he have a desire for revenge ? We know he worked with the Vault-Tec corporation.

The writers clearly make a parallel between his situation, someone fighting that much to stay alive when he knows the truth about this awful universe, when people in the Vaults who discovered it, decided to ki** themselves. We learnt also that it's Lucy's mom who opened the first Vault to the raiders, and from the outside. Or at least her key. I have multiple theories around that : she's from the outside and was kidnapped at some point, chose to escape without her kids ; or as soon as she had kids, they rejected her (they only needed kids) ; Lucy and Norman are kidnapped kids who were born outside etc.
