They are with them wherever they go and when they think they’re one with their own disease, the disease turns their back on the individual and the individual reminds itself that the disease wants nothing more other than to end that one’s life. It’s what sick people go through everyday with seeing healthy people and they have this burden of a sickness. It also symbolizes how no one knows what the sick person is dealing with (the cars driving past her in the beginning sign while she is hit and on the car). I’m guessing Gaga has battled a sickness and it conveying the hardships and mental struggle every sick person goes through.
