Also Duncan had been going to the hospital but Meg's creepy religious parents wouldn't let him see her. Reminder it was Meg's parents that were psychologically abusing Meg's youngest sister. So your idea that the letter could be about the pregnancy is still possible. The only hole I see is why didn't Meg's other sister say something to Duncan about the pregnancy when she needed help getting Meg's computer files before her parents found them.
