At first I thought it was another dream sequence, she wouldn't need 2 knives and that throat slit at the end to kill her, she's not a warrior. Then again, this fits in with Ivar's character.

I think it was most likely done by king Harald, since he potentialy stands to gain the most out of it. Especialy the way she was killed. Ivar will be glad, that she is dead. Which might finally turn Hvitzerk againts him, since he is going to blame him. But if it was one of the brothers, sending bunch of guys to slowly kill her seems more Ivars style. And I think Hvitzerk would care enough for her to do it himself, or at least make sure for it to be quick and as painless as possible.

I definitely don't think it was Hvitzerk. He seemed to genuinely care for her, so if he wanted to put her out of her misery he would have done it himself. I think he will find her and it will cause a greater rift between him and Ivar.

I stopped watching the episode 28 minutes in due to work, so this was a spoiler for me. It's fine, I don't care, all I care is: that crazy woman is dead.
