Remember that Otto Hightower is the grandfather of Aegon. By proposing that Rhaenyra marry her half brother, Otto is trying to solidify his family line being a future ruler. If Aegon is heir, that's great for Otto because his grandson will be King. If Aegon marries his sister, he will be King Consort and still have the Hightower bloodline close to the royal line.
