Sadly, Carol Ann Susi, who played Howard's mom, died in real life, which is why her character died on the show. Her real-life death was fairly sudden. She was diagnosed with cancer only about a week or so before she died from it.
I think you can really sense all of the actors' genuine emotions in that final scene when they toast Mrs. Wolowitz.
Here's some Big Bang trivia: in the final scene of episode 15 until the series ended, they had a small picture of Carol Ann Susi on the side of Leonard and Sheldon's refrigerator in memory of her (it's between the wooden frame and the front of the fridge, close to the middle of the freezer section).
And, the female employee of the escape room in episode 16 also played Bethany, one of the goth girls that Howard and Raj tried picking up in season 3 episode 3.

I remember when Carol Ann Susi passed.. all of the cast said that she had been like a mom to each of them. She was warm, funny, had a great smile and such a giving spirit! Kaley Cuoco & Johnny Galecki both mentioned how they had felt comfortable talking with her if they had a problem or just needed a listening ear, and she was always patient & loving, listening without interrupting, and offered great advice when asked.
When they were all sitting around Leonard & Sheldon's living room at the end, talking about her & offering a final toast, that was Carol Ann that they were talking about and the tears were very real.. the cast put up a photo of her on Sheldon & Leonard's fridge that stayed up for the remainder of the series.
Summing up I think what they all had felt about the actress, Kunal wrote at the time: "My heart is broken. I will miss your smile. Your spirit is forever with us." What a lovely legacy to be able to touch so many people with your warmth, kindness and big heart!🥹💗
