My favorite part of this episode is the moment David Boreanaz nearly breaks character after Cordelia sticks out her tongue. He bites back a laugh so hard and it cuts away when his smile breaks lol.

For what it’s worth, the Mr. Bill reference was pretty old even at the time. Mr. Bill was a clay figurine character from short films on Saturday Night Live back in its earlier years. At the end of every one of those shorts, the clay Mr. Bill would be squashed, while the voiceover of Mr. Bill shouted, “Oh no!” in a high pitched voice.

I really like this episode. It is sometimes very uncomfortable to watch, which is part of why it is so strong.
Another reason is the actress who plays Bethany. She's amazing at being both strong and fragile at the same time.
Also, Cordelia is extra fun this episode. "You can't fire me, I'm vision girl." "Stop flexing your manly boob muscles." "That narrows it down to people with friends. Where do we keep that list?" etc.
