Well to be fair, they might not outright say it until later, but that second question is answered in the first 8 episodes. As soon as they showed us Eren was inside that titan in episode 8 my first though t was "Oh? Well THAT is clearly what happened. Im pretty sure I know what caused this. I dont know HOW its possible but Im 100% sure WHAT caused it" and much later, they confirmed it.

I love how this show keeps insisting that losing wall maria was equivalent to losing a third of their territory when actually its a bit more, quite a bit actually because the km^2 (or miles) are not directly proportional.

At the beginning, Armin kept that one small blade to kill himself with. He would rather that than be eaten by a Titan.

Also wall rose (middle wall) hasn't been broken, only the gate of the city connected to the wall. At the beginning of the show when wall maria (outer wall) was broken the colossal titan broke the gate of the city letting them in there but it was only after the armoured titan broke the gate of the actual wall that humanity lost a third of there territory.
