As for you not remembering this from the bit of the show you have seen it’s because it didn’t happen in the orginal. In the original keichi is the one to go insane however Rika having a chat with him this episode prevented that however Rena had gone insane which didn’t happen in the original. That vision he saw and the opening of the series with him killing people with the bat was the original ending of this arc but due to some seemingly minor differences in events this outcome occurred instead. Also not sure if you saw the message after the credits but the next episode will be a new arc now.
