This really should be in the game's controls page I also had to find this out through people's comments on videos when I played the game for the first time

Sometimes entering a room will mark it as explored, when in reality you just went it and came right back out. Especially the smaller ones. I think. I really hope you don't consider this a spoiler... I wouldn't call it that myself, but I feel like what constitutes a spoiler is a very personal thing.
