The original scene has Rinko deciding to sleep over after trusting Honjo, with her taking sleeping pills so the scene can start.
I've always thought this was entirely unnecessary; this is the one man she's ever known and trusted.
She's never been in a relationship with a man, and doesn't know how men and women act like when they live together.
This is an opportunity for Honjo to convince her that mutual relief is something adults do in the absence of a partner, turning the sleeping scene Into one where he gets smothered and serviced.
The scene now also naturally progresses into mating, as Rinko learns more about taking care of an old man's needs...
