The creepy guy has been probably infected with a spiral-cursed version of the illness from the old lady's son called Cutaneus Horn, which is a real illness but not infectious. It can affect old people, people with HPV, or people with high levels of sun-damaged skin. It's a growth on the skin that occurs due to excessive keratin growth and it mostly looks like animal horns. The spring guy was the one who got hit by the car while confessing his love for Kirie in the previous episode, and the after-credit scene that same episode shows him leaving his grave. His nickname was jack-in-the-box.
