Shigaraki with the dog and his sister were a mistake the rest were all intentional, I'm pretty sure. From his own mouth. Very least I'm pretty sure the grandparents definitely were on purpose. Oh yeah and right before the family being killed his father realized he was a piece of shit and was going to do better but it was too late.
