I think narratively, the scene was about showing Cait that buying glasses is no big deal and she can't "behave wrongly", so having her fill out a form with her home address, for example, would have worked against that narrative, because she would've been pushed in the uncomfortable position to have to invent an address. And any additional paperwork would be unnecessary anyway if you pay in cash without an insurance. I get how the scene was amusing to you, but to be fair, it was done properly in the context of the game. And it was implied that the glasses cost a couple of hundred dollars, because the initial sum that Cait found in the library ($70) was said to not be enough.

I've spent many of my childhood years in an optometric office, and I spent seven years working as their billing specialist. And I'm laughing my ass off at this scene with Caitlin. A brand new patient walks in off the street without insurance or photo ID. There's no paperwork whatsoever. The whole thing costs a hundred dollars out of pocket. The patient has no medical history or older prescription, and the exam is over in only "a few minutes". It's just the one person running front desk, exams, and optical. Those old frames had damn well better be discontinued, because there's no other way that could be affordable with only a handful of bills out of pocket. I notice the optician upsold her on the lens coating, that's well and good. To be clear, of course none of this is a dealbreaker because it's a work of fantasy. Moreover, the general premise of the game and the established rules of etherwalking make any realistic portrayal impossible. But speaking as somebody well-versed and perpetually frustrated with the crippling bureaucracy and excessive overhead of the medical industry, my personal reaction to this is not offense but amusement. This is genuinely funny to me.
