I find the chorus of 'people need to remember this CEO was a person, with a family, and a life' hypocritical given the CEO didn't afford people the same empathy when making decisions about their business. I'm not saying I support what Luigi did, but I also think it's tone deaf to ask people to think of the CEO in ways that the CEO wouldn't think of people when they were alive.

I think this narrative that he can’t have sex because he’s in so much pain some how is the reason why he did what he did is nuts. People are assuming any of this is true about his inability to have intimacy. So much speculation. Maybe he’s just severely mentally unstable and is acting out for his own selfish reasons.

I had a similar situation after having my son and he was in the nicu my doctor was amazing and made sure insurance allowed me to stay in hospital as well so i could nurse my son as he needed while being cared for in nicu (it was a pretty physically traumatic birth on my body as well) I’m thankful your father is doing well and has you both to advocate for him. Lauren - your dark humor does not go unappreciated! This afternoon I was in our (healthcare facility) workroom and one of our less experienced residents was asking the attending physician how to appeal a treatment that United did not approve for their patient. Apparently, from the 8 criteria that United required to approve that type of cancer treatment, that patient only met 6. I spun my chair around and said, “I’m not trying to seem like a terrible human, but maybe this week they’ll be more likely to compromise on that…if you get what I’m saying…” 😏😏 then just spun myself back around to my computer! When you carry really heavy stuff all day long, sometimes a little snark goes a long way!

Luigi is an empathetic character for sure. As a 26 y/o that also deals w physical chronic pain, I see his pain. And billions of other people’s pain. But Dr. John’s analysis so far on the limited information he has on this case still sounds similar to A LOT of the other criminals that he has gone deeper into throughout HTC’s entire show. He’s truly an expert into how these people think and why these types of horrible things happen. Dr. John always has an incredible amount of empathy for victims and offenders alike. But the violent actions of 1 can have more ramifications than we could ever imagine for others. Justifying violence in any case is never going to solve our problems. I always learn so much by looking at the entire picture with you guys!!

SO concerning that he still had the gun. Somebody else might have ditched that so long ago. In our culture of toxic masculinity, he had everything, and possibly has lost it all. He was on top. He likely looked to that for his identity, he may truly feel like he is nothing. It seems like a very serious mental health crisis. He doesn’t want to look inward for two reasons, one, it’s too painful, and two, men don’t push it inward, they push it outward, to the more societally accepted emotion of rage. In my experience so many men seem to have maybe two emotions, satisfaction/happiness, or, rage.

A day or two ago, I realized that I was starting to worry for our Luigi. It was upsetting to see him trying to yell and looking so angry when he was being transported, and I’m like, there’s something more going on here. Can he handle what’s coming up? This case might actually make the history books, and it’s going to be wild. Also, he looks a lot like my older son, who is around the same age. My initial response was, oh no, he’s just a baby! It’s really interesting to hear all of this. I’ve only been listening to this episode for about 10 minutes, I feel like I’m getting it a little bit more. And I am worried for our Luigi. I think he might have a lot going on, and some of it has only just begun.

Seriously. My husband and I were talking about it last night. He pointed out that the politicians who could actually make the changes are swayed by $$$ as campaign donations. He could’ve helped in that way. Instead, he was selfish and honestly, I think, pretty bratty, for an oversimplified term. I fully empathize with having a medical problem and the possibility of it being made worse by injury/surgery. Not the same but I dislocated my hip and my shoulder in my 20s literally just by existing (it’s a talent, what can I say). I’m 39 now and can’t afford PT to fix my hip which has now come to include my hamstring and my knee. So I really have a hard time empathizing with someone who COULD afford to get help and chose not to. Even if it weren’t fully effective, there are options out there, even if like Dr. John said, it came down to getting therapy to help accept what happened and its impact.

I not only had immediately had images of Bryan Kohlberger flood over me when the initial photos surfaced… (I’m super intuitive with photos for some reason) but the rationale makes so so much sense. He has a young male brain- many don’t “finish” until late 20’s … and why why why couldn’t he have just taken out a paint ball gun with some sort of fluorescent paint to their corporate headquarters or even disrupted their conference peaceably…if he wanted to call attention… never ever ever should permanent violence be tolerated and now encouraged…and rooted for. I’m just sick about the public reactions. I so agree that this is his selfish rage at his own situation. I absolutely love the correlations with literature Dr John!!!! I’m reminded that Hemingway himself - well- throws of depression.

The potential of a sexual dysfunction component is really interesting. If that is the case, he could be technically classified as an “incel” as someone who is involuntarily celibate. Though, sexual dysfunction is not necessarily a reason society would stereotypically associate with incels, it would/could have the same result as the many other reasons society would typically use to explain any given man’s involuntary celibacy. Similarly we often see/hear about the incel communities online and these men being radicalized in some ways. Though they typically choose to blame women and act out against women, it is not dissimilar to the suspect (if he is later proven to be the killer of course) blaming the healthcare industry and acting out against it. The root cause of the behavior is the same, a man has difficulty sexually for one reason or another (amongst other perceived slights in life), and chooses to blame an entity outside of himself. Taking no ownership of his own potential contributions to this circumstance, he acts out aggressively and/or violently toward the subject of his blame. Very very interesting! An interesting view of the hetero male psyche. Though I personally can understand feeling the way he did even without sexual dysfunction, if that proves to be true, it just adds another layer.

Went through that with my Mother. She had a lumpectomy for breast cancer. Even brought her overnight bag. Oh no they said she was out patient. She started vomiting on the way home. We get to my house call Dr for new meds. Have to call a friend to pick them up.. can’t leave her alone. Set my alarm every 4 hours for her pain meds through the night. Great time had by all.

I was listening to Dr. Steven Hassan's live Q&A show today. He brought up Mangione & said he had seen posts claiming that rather than being left-progressive leaning as many consumer protection focused health care reformers are, that he was radicalized and a follower of the quack Robert F Kennedy, the manosphere and consistently factually wrong guru Jordan Peterson, and the like. He did not cite sources & I did not immediately find anything about this in a quick search. Lauren could you dig into this with your investigative talents? Unfortunately in our fractured and polarized & frozen political system, far too many people don't have any historical understanding of how the USA ended up with the most expensive, noncomprehensive, individualistic & corporate-profit driven health industry. This is despite amazing research science & tech breakthroughs & lots of overall health gains esp with public health, now widely underfunded & demonized postCovid. Far too many legitimately frustrated US residents are turning to a simplistic, vengeful 'burn it all down' mentality or culty wellness quackery online vs digging in to the hard work of reform. The US is also awash in bad "cheap" ultraprocessed food, in car-dependent-by-design cities and suburbs that cut healthy daily activities and mobility out of lives + also are huge costs to budgets. We spend far less on food and much more on sick care that pretty much every industrialized country. That ia not a coincidence. Yet so many people voted for more of the same due to... constant propaganda & scare media about the price of eggs? (note - millions of laying hens have been killed due to bird flu, plus supply chain covid chaos & hoarding, driving up prices, which is circulating more widely & may be the next deadly pandemic). There are a lot of really good medical professionals out there - including legit alternative practitioners - caught in a bad insurance & corporate ownership & consolidated, high-profit-driven system that seems harder & harder to change. Many of my fam & friends are nurses, doctors, PTs, naturopaths, and more. They often hate the system too - well except my friends who work for the socialized medicine branch, i.e. the VA It has come a long way thanks to reformers. So I am sympathetic but It is scary. Dr. John please address both issues.

I have a question… I have chronic back pain as well. Even with medication I never have a moment pain free, ever and I’m not a candidate for surgery at this time. I would never consider murdering someone because of it, I would be terrified of being in jail because I can’t imagine that medical care and opioids and muscle relaxers probably wouldn’t be allowed? Are prisoners medical needs like that taken care of?

Thank you for your podcast on this issue. We are definitely in need of a better health care system. I have also been denied coverage on knee & neuropathy issues because I chose the natural way & had to pay out of pocket. Was I angry, yes, it was far less expensive than going through surgeries that the insurance companies would have paid, but it would never cross my mind to kill someone over it. However, I am a little perplexed as to why he went after insurance CEO. If his injury was a result of a botched surgery why go after insurance? Was he denied coverage? To me it just sounds like he was trying to make a name for himself. He had everything going for him & this one horrific instance has ruined his life forever. Such a sad commentary.

Just fyi, before the surfing incident, which was his fault, he had a chronic spine condition since childhood, which was not his fault. They say he got weird after the spinal surgery, and his parents had not seen him in 6 months and reported him missing in Novemeber. I really had to wonder if the spinal conditionor the spinal surgery could have affected his brain given the amounts of nerves in the spine? He should not be killing-but there may be some medical issue with his brain since he definitely did this out of character.

Ty for the follow-up and IRL IRT insurance situation of Dr. John's dad. Yes, it can feel like the system is so hopelessly rigged that individuals are powerless. Unfortunately in the US, awash with guns both legal and non, some people, especially men, try to exert power by horrific criminal means. Like the Delphi killer, the gun gave him the real opportunity for power over others. If Mangione had no access to a gun, maybe he would have shown up at the shareholder meeting and thrown a pie in his face - this happens! - or punched Mr. Thompson outside the hotel, causing injury but not death. But if anything, the US will just get more and more violent and awash in guns as any effort at gun safety legislation is thwarted by lobbyists, activists, and reactionary courts. The fractured us populace + political system seems unfixable and rigged. Yet so many US voters fall for the corporate & billionaire PR propaganda that holds the whole system hostage. The Affordable Care Act was an incremental improvement that left private insurers in control becasue the Dems did not have enough votes to create a better system and the Medicaid expansion was flawed and ruled state by state option by the majority-conservative, business friendly Supreme Court. Even then, the PR machine attacked "Obamacare" with accusations of death panels, delays in getting appointments, and the absurd "socialism" label. And the Dems had historic House seat losses and lost any appetite or power to improve the health insurance situation. And Repulbicans who overhwhelming are anti-consumer-protection & anti-regulation and pro-privitazation of all public services and goods jist won all 3 branches of government. So anyone who voted for Trump and R politicians and actually thought he would fix it is deluded. They are now a political cult. Yes, it is all political and trying to avoid politics instead of reforming it only helps the wealthy, the corporations, and the increasing wealth inequality & destabilization of the USA.

Thoughts are with you Alison. I have severe complex PTSD medical trauma from the first 12 years of my life. I also feel empathy for how pissed off this guy must have been given what was covered in the first episode of this case but at the same time I can’t fathom resorting to shooting someone dead because of it. I find it odd- if he’s anti capitalist and idealist- that he was in McDonald’s. It’s a symbol of corporate greed. I know lots of people who won’t go there based on ethics. I agree re death by cop or self deletion- why else keep the weapon and have the manifesto handy? But I think maybe he thought they would realise his name - or that he would have some kind of warning -or seeing police cars etc I’m not sure if the police who approached were in plain clothes or not. As someone with endometriosis, pcos, pudendal neuralgia and an injury sustained giving birth to the only child I was able to both conceive and carry to full term, I feel for anyone with severe chronic pain and secondary mental health issues it causes, but F this guy. UHC, Kaiser in CA, Cigna are the worst. As a provider in private practice I like so many other health care providers stopped being in network providers. They are notnotorious for denying claims. Their have poor reimbursement rates for providers, and make consumers and providers jump through hoops to appeal claims that should be covered. The companies profits are staggering, while consumers pay for ever increasing premiums and providers are forced to accept low reimbursement rates to be in network. I think that is why people are supporting Luigi as represents the anger of many caught in the insurance industry greed for more profits. I do not condone violence of any kind. It is unclear if the condition he had was congenital prior to the surf board incident. I do not think (unless he was advised by medical professionals) that it we need to blame him for his surfing accident or say he is responsible for his spinal injury. I am not condoning what he did. I just think we don't have complete verifiable information. So before we say he is not taking responsibility for a choice he made that was the cause of his condition, it would be helpful to learn more.

I watched the interview of his friend and housemate when he lived in Hawaii. He said that Luigi was a health and fitness enthusiast. He shared that they did workouts together. But he was diagnosed with a painful spinal condition that was debilitating. Some days he could work out and the next day he could not move and was in extreme pain. Luigi said he could not date or have sex. The surgery he had failed. The friend said he left Hawaii and returned out east. He isolated from family and friends. His own mother reported him as a missing person as he did not respond to multiple attempts by family and friends.

Outrageous that the hospital is insisting that you take your dad home given he is elderly, had heart surgery and needs monitoring! What if you lived far away and taking him home is not feasible? The answer is they could not discharge him as the hospital is liable for his care. Most families do not know this and are pressured by the hospital to take their family member home and be responsible for their care. I learned this as a health advocate

FYI I just went thru the exact same situation last month regarding my heart and a procudure. I was to be kept overnight as I had noone to pick me up and then observe me overnight. However about an hour after my procedure the nurses starting to tell me to get ready to go. After 2 hours I was out - took a cab and came home alone which was very concerning to me as they had STRESSED I needed to not be left alone. SMH

I am glad they are going to keep your Dad for the night. He should be there to recover and rest and to make sure everything is going good with the surgery! Your Father anticipated staying and just them saying he was going home could cause him to be upset and affect his heart and blood pressure! Wow! On another subject I want to say as someone in my early twenties with two very small children. One a year and the other three and a half I had a pretty routine surgery that went bad. Really bad, that led to several surgeries and me never being able to have any more children. I had a rupture ectopic pregnancy and miscarriage trying. I was so overwhelmed but was determined to live. At my lowest weight with one more surgery to face at 89 pounds. Now the thing is there were so many issues. This was before family leave act. My husband used all sick time, vacation time and actually gave up a career position to keep his previous job to keep medical benefits. There was malpractice and healthcare abuse and on and on. Eventually legal action and it took years that was hard on a little family. Although there was plenty of people who I could blame and take out my frustration I thankfully eventually healed. I never thought of killing anyone. I did think those responsible should be held accountable but legally. Sadly I suffer with many health issues including severe back issues. I still carry on. Pain everyday! I know this young man is young but he now has not only ruined his own life but taken a life and devastated families on his side and his victim. Death and grief is something you never get over. I lost my Son and Dad in 2020! You are never the same, you just learn to live with grief as the price you pay for loving someone so much. I am glad you are on Patreon addressing the health care aspect and the suspected shooter. Once again why people do what they do? Being physically, mentally and emotionally broken. Keep up the great work and in this time of fast past info. Any and all info. is important. You can always make a correction. When I was in nursing school over 30 years ago, I did take care of a young man, 19 years old who had broken his back in a motorcycle accident. I have often wondered if he was able to recover from his injuries. He did make inappropriate comments to me which at the time, as a 21 year old student I didn’t fully understand how scared he must have been, if he couldn’t have sex again. I think for a very young person of either sex but especially males, this must be devastating. I think this killer had a mental health change over his pain and inability to move on in his recovery after having been well on his way to a promising career in computer science. I think he became radicalized and isolated himself from friends and family. It’s a tragedy for all of the families involved. Praying for Mr. Thomas’ family especially his children.

John, I also think he probably feels outraged about the profit listed by this company - $75 BILLION aus dollars. Apparently it’s one of the leading causes of personal bankruptcy in usa - being unable to pay for treatment etc if you are diagnosed with an illness like cancer etc. Appalling way for him to deal with this, but I feel like the profit they announced would have further outraged him. We feel so luck here in Aus that Medicare takes care of ALL medical fees for everyone. Xx

It seems it goes back to resilience or lack thereof. If we look at Christopher Reeves who was injured riding; his post injury journey was so different. I’m not being fatalistic but it really seems our society is raising a lot of folks that lack resilience, and that’s not to say I have no empathy for the injury etc. just thinking through differences in how people handle such things.

I love Dr. John’s insights. His take on the suspect shaking makes sense. He’s so cocky and full of rage every place else, but in that moment, he was shaking. It makes sense to me that he was thinking of pulling out a weapon and perhaps dying in the battle. My ex-husband has same back disorder as a suspect. He’s going to have second spinal fusion in January. It definitely changes their personality dramatically. The pain and the pain meds.

Also, re: word counts (I'm a book editor so I look at them all the time), the word count is calculated a little differently from system to system for some reason. Even from a client's Google document to Word, there's some discrepancy usually.

Sorry if this is tmi, but I dated a guy who had sciatica and he had sexual problems. He was in his 40’s though so I can’t imagine a twenty something going through it. Obviously people have dealt with worse and never murdered anyone.

I had the same surgery (looks exactly the same, T12/L1 with pins above and below, rods along both side, after a freak car accident in August. If anyone has questions about what this is like, I'm here. I've only just started PT today and my therapist was expecting I'd be way, way worse off.

i had similar injury, without the surgery he had @ 20ys old. I got help after 4 years of chronic debilitating pain, naturally non surgically. I became a chiropractor as a result of this deep wounding and I learned BioPhysics the most advanced form of chiro, cuz it helped me return to normal. He has a long road ahead to heal. I would guess he is not impotent, he’s injured. Pain changes you. He feels compromised absolutely!!!! I prayed to GOD from the depths of my soul to show me the way OUT, and he did. This guy is weak, his spine collapsed, he is in crisis and vulnerable to further injury if he simply bends forward to lift a pen from the floor. He may not even know how this happened and it may be blurry bc he’s an athlete!! This is a huge blow to a Penn State grad w a 6 pak abdomen!

I have anxiety and part of it is anxiety around medical situations. My mom and dad had constant health problems, life altering surgeries, and pain throughout my childhood. My mom, dad, and now my brother have all had heart attacks and/or heart bypass surgery before the age of 50. My dad had 5 bypasses and my brother had 4. Seeing this stuff as a kid was traumatic as a kid and I'm hyper vigilant about my health. I could see a failed back surgery for him triggering a complete tailspin and breakdown. I told my husband last night 'i get it...I totally get it.'
