It's called Marie Antoinette Syndrome, and it's a real thing that's been documented plenty of times and with successful experiments on mice. Which author are you referring to by the way? (Berserk had this happen around 2004, and Black Jack started in 1973, there might be earlier examples I don't know off the top of my head.)

It relies heavily on the sentiment "Pitou wouldn't remove her En for any reason except to heal the King", but they never asked the question "what if the King ordered her to?" And, as we saw, there's at least one good reason for him to do so, and probably many more besides. That being said, Killua's conclusion "something fishy is going on in the palace" is still valid no matter the reason.
