I looked at the comments, and since no one mentions it Dr.Melfi is the one who left the note for Tony. Its a perfect quote that sums his character up "Sometimes I go about in pity for myself, and all the while, a great wind is carrying me across the sky". Tony hates looking at it because it is him; And no i do not think she came to the hospital herself, that's to say she gave it to Carmela at the therapy session offscreen There is a great video that explains this quote in more depth that I'm not gonna go into detail here, but some points from it. Are the following, the "the great wind" in the quote is his father, and his mother, . Two extremely vital figures that shape Tony and eventually lead him to this life. Another example is when Tonys father is literally driving the car in his dream. LITERALLY steering him on a road. God this show is gold, and some really don't find the diamonds until they look even deeper. Thats the thing though this is just my take on it. One of the best things about Sopranos is that a lot is open to interpretation.
