Exactly, I love the ambiguity of this show. Especially during therapy sessions, either Melfi or Tony asks some very interesting questions, maybe some big questions about life, but then we immediately cut to another scene, abruptly ending the therapy scene and leaving us in ambiguity. Just like in real life, most things are unresolved and just left in mystery. All we can do is give our own answers and meaning. There are so many examples like this throughout the show, especially in later seasons. The Sopranos is probably the most profound TV series ever made.

A neat thing about the structure of the episode is that the title refers to each of the three therapy scene tony has in this episode.
- first one was him in denial telling himself that jackie is gonna beat the cancer
- second he lets his anger and frustration get the better of him and he lashes out at melfi
- third after seeing how bad jackie has really become, he becomes more accepting and ponders more philosophical questions.
This show also does a good job of keep things pretty ambiguous and not spelling things out exactly when it poses some questions. At the end when melfi asks tony if he feels like frankenstein, some shows may just have tony say "yes" or "no", but they chose to just show him react emotionally to meadow singing but still not give an explicit answer as to what exactly he's getting teary about even if we can make a good guess. A better example was the first episode where they reveal that he links ducks to his family and that he feels dread about losing them, but then stops short of laying out what exactly that dread is when melfi asks for more. The episode actually gives some hints and foreshadowing but it's not gonna be too clear on a first watch and can have multiple meanings. Future episodes will have complex and ambitious examples of this.

Worth mentioning: This is also what Charmaine Bucco is worried about. Tony and Artie are friends, but if Tony gets involved with their business -- as an investor, or even by giving them "free" cruise tickets -- that would (probably) lead to a situation where Tony asserts more and more control over their business.

You asked what the mob "does" to make money, and this episode pretty much nailed it: What they do is extort money from OTHER businesses.
Generally speaking, they do some sort of initial "favor" for the people who own a particular business, and in return they get a portion of the income from that business going forward. Forever. Garbage collection is the quintessential mafia business, which you saw in the first episode, and now they're muscling in on a motel. After this small bit of strong-arming, they will now get a few hundred bucks from this guy every week. That doesn't sound like much, but if you have 50 business like that it grows and grows.
They do the same thing with plenty of other organizations that aren't "businesses" per se, but it runs the same way. Labor unions are one of the stereotypes, where the mob controls the union and gets a kickback from the "pension fund" or something for every contract they have.
They will also do all sorts of other individual crimes (we saw Christopher doing truck hijacking, and Tony's friend Hesh does loansharking). But the "business" of the Mafia is making sure the other businesses keep paying week after week.

As one of the 2% that is here for your reviews and discussions after the episodes probably more so than the actual reactions to moments throughout please never stop your long reviews. So glad you're already so into this show, it really does hook you early and drag you along for the crazy ride as you get immersed in these people lives.
It's not always a particularly easy watch but it's so superbly executed that like The Wire you cannot stand to miss a second of it. Hope you all are doing well

In my opinion this is when the sopranos really gets going. The first two episodes were almost like introductions to the world and establishing the characters. From here on I think it starts getting really good really quick.
