I imagine it's a case by case basis. Dreamkeeper physiology is hugely variable. Some are birdlike, reptilian, mammalian, fish, or sometimes none of those.

If you ever look at dogs and cats when they're rolling around being derps....you can't see the nipples on male animals unless you really look, pushing the fur aside. Females, especially those that have given birth, have ones that are way more prominent, though they're not typically grown out as in humans.
