That is a fascinating insight into this sad event, and in particular the background to the sentence being overturned. It strikes a chord with me. I was for many years a union rep in a safety critical organisation. I had several cases where a member had been part of a disciplinary process for making a mistake. I always started with the statement that " Nobody should be disciplined for making a genuine mistake" It usually helped management focus. Then moved onto training, monitoring, workload, rest, previous incidents, previous performance etc. But I felt starting that way generally helped make the case.
