Always with the clutch epic entrances! And poor Megalo being beat up from the inside-out! In regards to Queen Otohime, I think she knows who it was that shot her and that's why she specifically asked, no matter WHO it was to not hold a grudge against them. She has a really powerful observation haki that lets her hear and experience people's emotions and true thoughts/desires. I bet she could feel Hordy's malicious intent from a mile off. The same way that Luffy can instantly judge someone's character the moment he meets them. The same way Koby was (uncontrollably) hearing everyone's "voice" being snuffed out in the Marineford war. Otohime had this same power and although she may have been aware (or maybe not until after the deed was done) of what he was thinking, she wasn't quick enough to act or dodge the shot. Well, that's my theory at least anyway. I don't think it's ever fully explained but kind of implied. You're about to witness some of the coolest and most impressive feats of strength in the anime so far, in the upcoming battle! Prepare yourself! I am so PUMPED!
