Mitigating die-off on the fungal protocol

Before we dive into exogenous strategies for mitigating die-off reactions in the fungal protocol, there are a few things you may want to try first before considering the addition of more supplements:

Try reducing the dosage of the supplements listed first. Typically, this involves simply cutting the dosage of the anti-fungals in half (one capsule of caprylic acid and one capsule of enzymes). Depending on how you react, you can try splitting this up even further, with one serving of the enzymes in the morning and one serving of caprylic acid in the evening.

It’s normal to experience some die-off reactions; we just want them to be tolerable. For example, loose stools, moderate to mild gas, and some bloating are normal signs of yeast and bad bacteria die-off. We’re not looking to entirely eliminate all symptoms, as these are signs that the protocol is working and that the body and microbiome are adjusting to a new set point. If symptoms are severe (such as severe nausea, brain fog, diarrhea, or possibly some mild anxiety or restlessness), simply reduce the dosages listed until symptoms are more manageable. Again, they don’t need to be entirely absent. We just want to mitigate the big stuff.

If you tend to be more on the constipated end of the spectrum, you might benefit from adding some of the motility aid products listed in the fungal protocol (like MegaGuard or ginger root) to support the proper elimination of toxins before considering binders (the stuff we’ll talk about here). This approach may solve the problem for you.

If you’re still experiencing significant die-off reactions with lower dosages of the supplements listed, consider doing the protocol in reverse. Start with the Just Thrive probiotic for one month (beginning with just half a capsule to assess tolerance), then add in the anti-fungals for another four months.

What is die off?

The die off reaction, commonly referred to as the ‘herxheimer reaction’ occurs when a large number of pathogenic microorganisms, such as bacteria or fungi, are rapidly killed off, causing the release of various toxins into the body. This phenomenon often results in a temporary exacerbation of symptoms as the body aims to eliminate these toxins.

When pathogens are targeted and killed, particularly through antimicrobial treatments, they undergo cell death. This involves the breakdown of their cellular structures. These are components of the outer membrane of gram-negative bacteria, primarily lipopolysaccharides, sometimes referred to as LPS or endotoxin. When these bacteria die, these endotoxins are released into the bloodstream where they trigger a strong immune response. This can lead to symptoms such as gas, bloating, nausea, diarrhea, fever, chills, and fatigue.

For most individuals, die off reactions are mild and transient, only lasting a few days. However, GI symptoms like gas and some looser stools can often persist on and off for a few months. This is very typical.

These symptoms are often mistaken for a worsening of the underlying infection, but they are actually just a result of the body's efforts to detoxify and recover from the sudden release of these bacterial and fungal toxins.

Supporting the bodies ability to detox

While all of the categories listed below will be relevant regardless of the form of ‘die off’ you’re experiencing (particularly many of the things listed in the liver and binder section) I have labeled these in relative order of importance based on the presentation of different die off symptoms. This will help you prioritize the right supplements based on your primary issues. If symptoms are systemic and numerous consider at least one of the binders listed as well as one of the liver support supplements.

chills, fever, fatigue, brain fog — support the liver

The liver plays a big role in detoxifying the body by filtering and eliminating harmful substances, including toxins released during the die off of bacteria and fungus. The liver is essentially the body's primary filtration system. It processes blood from the digestive tract, filtering out toxins and waste products. The liver converts harmful substances into less toxic forms. This involves various biochemical reactions, including oxidation, reduction, and hydrolysis, which transform these substances into water-soluble compounds that can be excreted. The liver also produces bile, which aids in the digestion and absorption of fats. Bile also helps eliminate certain waste products, including bilirubin and excess cholesterol.

You can support the liver by consuming cruciferous vegetables (like broccoli or preferably broccoli sprouts for the added sulphurophane, which is great at aiding in detoxification) and other sulphur rich foods. Doing epsom salt baths 3-4x a week is also a great way to provide the body with bioavaliable sulfate needed for detoxification. This is particularly helpful if you have hydrogen sulfide SIBO or tend to get bad gas from sulphur rich foods like eggs and cruciferous vegetables.

TUDCA, NAC and taurine (in this order of importance if aiming to budget supplements) are three of the best nutraceuticals to support detoxification on the fungal protocol, particularly due to their ability to support liver health and bile flow. TUDCA in particular is great for supporting bile flow, which will also improve GI motility. If you’re someone who tends to occupy more of the ‘constipated’ end of the spectrum, TUDCA is ideal. NAC is particularly helpful for mitigating acetaldehyde release from fungus as well as clearing mycotoxins. If you’re getting lots of fatigue or brain fog on the protocol, this would be an ideal choice. NAC will also support the production of glutathione, the bodies ‘master antioxidant’ needed for liver support and binding toxins.

TUDCA — bodyBio brand. One serving daily before or with any meal.

NAC — 600-1800mg daily (play with a dose you feel good on) with morning meal. Don’t take at night as it can be stimulating for many. BulkSupplements brand

Taurine — 2-3g daily. BulkSuppllements brand

Gastrointestinal (gas, bloating, diarrhea, loose stool, stomach pain, cramping, nausea etc) —- take ‘smart’ binders

Enterosgel is an oral intestinal absorbent designed to treat gastrointestinal conditions, particularly acute diarrhea and irritable bowel syndrome with diarrhea, it effectively binds harmful substances in the gut like endotoxin and even heavy metals and prevents them from moving into the bloodstream where they cause systemic symptoms like chills, fever etc. As Enterosgel passes through the intestines, it adheres to these substances, allowing them to naturally be expelled from the body through bowel movements. Importantly, Enterosgel does NOT absorb water, vitamins, or beneficial bacteria (unlike many binders like charcoal, clay and zeolite) making it a safe option for detoxifying the gut.

Bovine serum immunoblins are another fantastic binder that helps to suppress gastrointestinal inflammation while binding to mycotoxins, endoxotons and possibly faciliating the removal of some pathogens themselves. These are listed as one of the most important binders in the fungal protocol. They are considered an ‘optional’ supplement but are useful for many.

Both engersogel and BSI can be dosed very high without any side effects. This tends to be ideal for those suffering from more severe die off reactions, particularly in the early stages of the protocol. Entersogel is best taken on an empty stomach, 1-2x a day at 1-2 tablespoons. It can be used in the morning or in between meals. BSI can be taken anytime of day with or without food. Anywhere between 1-5 servings daily can be helpful, particularly for acute diarrhea and loose stool.

Body aches, chills, worsening of skin issues, exaggerated inflammatory or immune responses — use pro-resolving mediators

Pro-resolving mediators are a powerful anti-inflammatory that help to promote the resolution phase of inflammation. Instead of blocking inflammation entirely (which is an important component of mounting a proper immune response) pro-resolving mediators help to resolve the source of the inflammation itself (bacteria or viral infection, endotoxin, clearing inflammatory proteins,) by upregulating the anti-inflammatory immune responses. This is done while also promoting repair and waste clearance. SPM’s promote phagocytosis, which is a process by which our cells engulf or ‘eat’ particles such as bacteria, other dead cells, and debris. This is an important part of the immune system's response to infection and toxins. In the initial phases of die off, using higher doses around 4-6 capsules daily is highly effective. Lower doses (2 capsules) can also be effective and can be taken as needed on a short term basis ro manage residual or lingering symptoms.

Anxiety, depression, restlessness, poor sleep

Some individuals may notice an increase in autonomic arousal during the fungal protocol. Increases in stress, anxiety, and a slight exacerbation of depressive-like symptoms are the least common die-off reactions but can affect some individuals. Its best using some binders as well as liver supper first. However, some of the things listed in the stress article — particularly psychobiotics like Just Calm — can be extremely helpful for managing neuropsychiatric based herx reactions. Just Calm is best dosed in the evening (ideally 4-5 hours away from your Just Thrive or megaspore probiotic). JT can also be very helpful for improving both sleep duration, onset and quality. NAC may also be particularly helpful for those dealing with anxiety as well other cognitive symptoms.

Miscellaneous

Supporting other detox organs (like skin for elimination of water soluble toxins) as well as the lymphatic system is also important. You can do this by

1. Work up a good sweat. Long, shirtless or minimally clothed walks (aim for 60-90min 4-5x a week) tend to be great for this as red and infrared light form the sun will improve cellular redox, pushing out intra-cellular toxins. Riding a stationary bike also works. The heating effect from red and infrared wavelengths will stimulate sweating (natures free infrared sauna), allowing these mobilized toxins to be excreted through perspiration. Be sure to shower immediately after

2. Any form of movement or exercise will support lymphatics drainage. Particularly exercises like rebounding on a mini treadmill or even doing low intensity plyommetrics like jump rope. Vibration plates can be useful as well if you have them. Use a brush during showering or an exfoliating device to help remove dead skin that traps toxins and waste. Particularly on areas like the upper and mid back as well as the arms.

I believe he recommends the Metagenics brand SPM which is currently rebranding their SPM product. The old one is 500mg per capsule whereas the new product is 1g per capsule. I would assume when he's saying 4-6 capsules it's relating to the 500mg per capsule dosage.

Any recommendations of SPMs or dosage? I’m seeing capsule with anywhere between 500mg to 1g per capsule. And if we’re already taking fish oils, should we adjust dosage of SPMs based off of that?
