No SERIOUSLY! (UPDATE)

I've asked several people to use my commission form and so far few have used it. I'm asking that at least one of you send me a TEST to make sure it's working. Please put the word "TEST" somewhere in the form so I know it's only a test.
Also, why is the commission form not being used? Please be honest with me, is it hard? Do you not like it? Please give me some feedback so I can change it. I'm serious, I need to know how to make it more appealing to those of you who want commissions, so PLEASE tell me why you aren't using it so I can change it to be more appealing.

Okay, I'm not exactly sure what happened to my form but I now know why it isn't working. It will be down until I can build something much more reliable. Sorry for the issues.
