The answer to why the sword doesn't deem Regulus worthy is because Regulus is a complete trash scumbag and is incredibly weak without his authority. The sword has unsheathed for opponents weaker than Regulus before but Regulus was given his authority. He was a weak person before that, even Subaru would be able to defeat him with no effort. Also, the sword even if it was drawn would do nothing because of Regulus's authority so that would be pretty anti-climatic. In fact, the sword being sheathed gave Reinhard an advantage because the sheathe of the sword is indestructible and could withstand Regulus's attacks. Also they skipped some content but the reason why Reinhard took so long to get back initially is because Regulus let out a sigh which caused Reinhard's leg to get chopped off from Regulus's breath, overriding Reinhard's divine protection. After that, Regulus makes him fly through buildings. He was being healed by a lesser spirit while Subaru and Emilia ice skated. The anime went with a more comedic approach but the light novel showed Regulus as a very scary threat that even Reinhard is unable to damage. Lion Heart basically keeps Regulus's body in a state of frozen time. He doesn't age, can't get wet, or take any damage since his physical body is frozen in time. Its like when you hit the pause button in a game, your immune to everything happening in the game. This also makes it so Regulus is immune to all laws of physics and concepts of the world since he isn't restricted by time. This allows him to shoot projectiles that destroy anything in its path, take zero damage from anything, apply Lion Heart to things he touches like water and being able to launch Reinhard to the moon. Very busted character and the only person who could of figured out his ability is Subaru due to his knowledge on stars from the real world.
