You noticed that the movie is dedicated to Robert DeNiro Sr by his son, who not only played Lorenzo but also directed the film. But it's actually the story of Chazz Palminteri (Sonny), whose real name is Calogero Lorenzo Palminteri. He wrote it as a play, based on his own childhood, and performed it onstage as a one-man show. He sold it to DeNiro (who wanted to direct the movie and play Lorenzo) because DeNiro was the only one who would agree to let Palminteri write the screenplay and play Sonny.
