I'm sure you've gotten a lot of replies regarding it on YT, but to put in a bit: Companions are more along the lines of courtesans or peak-era geisha. There are both whores and Companions. Companions are held as high status people because they are among the top educated and health class - one where those things are maintained at a guild standard across the entire Alliance. ...making them very impressive to see to those on the OuterRim/Border Planets where education is highly limited and living conditions/health is questionable like in this episode was seen. The social dichotomy seen of esteem vs disapproval, especially with Mal's personal disrespecting moments, will be made much clearer before long. You'll get their status and why rounded out more as time goes on :)
