In general I try out different morph looks, usually pushing for fuller/fatter until I think its fine. Bigger is better, but its harder to keep up the realism the further you push it. For this image, I wanted it to look like the fat is sagging in the opposite direction since she's hanging upside down, so her cheeks are doing a bit of an anti-gravity, sagging up and obscuring her visio...
