In case you were wondering, I don't consider everything outside of the center of Hell to be empty. It's just that I wanted to keep the readers' focus on the middle. There's certainly some Demons who were left behind to watch over Medians and such.
The gray blobs that are most abundant around the water are dead (or at least, dead-looking) trees.
