I'm a bit late to this, but to elaborate on Brandon's death; During filming, they sometimes used rounds with a visible slug but no gunpowder or primer, so when the camera filmed the weapon an actual loaded bullet could be visible. On one of those rounds, the slug (or just a fragment of it) fell out of the bullet and remained in the chamber. They used the same gun later on, loaded with blank rounds. The exploding blank was enough to force what was left in the chamber out as if it were a bullet, and that's what killed Brandon.
