Takamuras weight cutting is so extreme because in Japan there's no real weight class for boxing above middleweight and his everyday weight is 4 weight classes ABOVE that. He literally has to drain every fluid out of his body to make weight. Usually IRL there's not a 4 weight class difference for the weight you have to make but it shows Takamuras drive to be great.

yea but then after they weigh in, they'll eat some food and get some weight advantage over the smaller guy

I have a cousin who was part of the national judo team and most of the professional sports that involve a weigh-in (to fall within a specific weight category) share this harsh reality, and it's harsher when someone tall like Takamura tries to meet a weight that's below their expected weight class.
One time, my cousin was past the weight limit for his class by like half a liter of water (using that measurement to make it easier to understand, since we use grams for weight), so the coach made him run for almost 2 hours to sweat off the excess water weight and barely made it (He looked like pure muscle, super little body fat %). When you're that low on fat and excess water weight, even drinking a bottle of water can make you bloat like crazy, it's super difficult to maintain class weight.
It's harsh, and a bit dramatised in Hajime no Ippo, but not fake at all!
