i get what adam was saying about that scene being a metaphor for SA, but i think that would be too simplistic a way to look at it. from creature-hikaru's point of view its more like unloading an emotional weight or connecting instantly on a deep level beyond human understanding. like reaching into the soul of the person you love to seek comfort. and the reason yoshiki switches from "this is gross" to "this feels good" is because of his internalized homophobia and being afraid of/feeling guilty about having that level of intimacy with another guy, when in reality thats what he's seeking too. its definitely erotic on some level too, but its a whole mess of things on top of that
