Background story: X-Men get wiped out during an intervention in Genosha. Apocalypse inherits the mantle of mutant savior and revenges the nuclear destruction of Genosha by wiping out most of the Avengers. The US Government conscripts multiple metahumans into become Omega Sentinels. In this picture, it's Sue Storm. She was forced to surrender to the process or else her son Franklin (a mutant) would be at risk. Apocalypse after losing his horseman increased his ranks by forcibly conscripting others. Spider-Woman becomes his avatar of Pestilence.
