It threw me off, I really didn't expect him to pull the trigger hahahah my mind was blown! but yeah, really liked the direction they went with this episode :)

I too appreciate that they didn't demonize people with mental illness. Your reaction to Sam pulling the trigger made me laugh so hard. You're right that in his right mind Sam would never pull the trigger, but here his feelings of frustration and anger in regards to Dean issuing orders and always following John's, were magnified, big time. I was also so surprised the first time I watched this episode because I thought like, "the power of brotherly love" would win out and Sam would be snapped out of it. But - NOPE!! LOL! I appreciated that they kept things consistent, that the doctor's control made EVERYONE kind of rabid. Very surprising, but awesome.
