They are either born that way (people like pdos typically (although they can be developed later in life, it's just rarer)), or things in their life that they have been unwillingly exposed to, mainly in childhood. (things like BPD). A paraphilia is not inherently immoral, but acting on them always is. Thats why we need to offer pedophiles (that have never acted on their paraphilias (I want to be very clear that I mean people who NEVER ACTED ON THEM, because people who did are still absolutely responsible for their actions and disgusting people)) and people of the sort psychological help instead of meeting them with disgust, because that is what will stop them from hurting potential victims. It may sound weird, but its the best course of action to avoid harm. Pedophiles who have been treated have a much lower offence rate, that nears 0.
