Pagan's constant stuffing of himself and transferring the weight to Delirium eventually starts to add pudge that doesn't go away no matter how much he's "thinned down" by Pagan.
This is reflected in Pagan's growing waistline, despite having Delirium's belt (meant to keep oneself looking thin regardless of weight).
All in all, in means only one thing. Pagan is eating too much, even for a Glutton.
