Cloverworks did a great job with this show. In some parts is even better than the game. 1. There are three ways for a Servant to manifest in the world. Being summoned by a Master, being summoned by the Grial as a chain reaction or as someone to administrate the war (like the Ruler class), and finally being summoned by the Counter Force to protect the world from a threat. Since the Heroic Spirits are disconnected from the Space-Time contuinity, they can be summoned to every age if they have already died in their time. The problem with Merlin is that he will never die, therefore he will never ascend to the Throne of Heroes (the place where the soul of the Heroic Spirits reside). That's why Romani refuses to believe he is there. 2. Avalon is the distant Utopia where King Arthur arrived after his death. This place is located in the Reverse Side of the World. A place where every Mystic evacuated after the end of the Age of Gods, since they cannot exist in the world anymore. You shoud recognize the place, is the place where Sieg takes the Grial at the end of Apocrypha. Only Mystic can access that place and just as the Throne of Heroes, is disconnected from the normal Space-Time continuity. Also, I recommend watching Kara no Kyoukai (The Garden of Sinners) after finishing with Fate if you want to continue your adeventure across the Nasuverse.
