Genius like this doesn't come along everyday. If you're lucky, you get to experience it while it happens. Yes, this was one man's idea, one man's world. He gets inspiration from reality, and then it changes it to suit the reality for One Piece. Water 7 is Venice, it has environmental issues directly related to a change in sea level. Skypeia is the battle between indigenous peoples and those who have chosen to colonize them. Both of those are important to the greater story. The story is full of foreshadowing and some pretty blatant hints of what's to come, as well as the beginnings of ideas that he had yet to flesh out. He uses the real world for inspiration. You'll see geology and geography - we have Venice right here - but also culture and government and history and mythology. He creates characters that have depth. Not just our main characters, side characters. He can make you empathize with a character you met 5 minutes ago, and you won't forget them because he doesn't forget them. He has people helping him with the research now, he's had to for health reasons, but this is still one man's idea. He has the final say.
