128 - Entrance to the Far Realm

She was tricked by a powerful being who managed to open a rift to the far realm. The creature also took control of the mage and now aberrations and unknown horrors are coming in from the terrible plane.
That's a synopsis of the attached adventure idea. It includes specific ideas for encounters and the background lore off how this mess came to be.
